All applicants who wish to apply to stay in Sarawak under Malaysia My Second Home Programme must submit the following documents to One Stop Agency:

| 4) | Personal Bond Form (affixed with RM10.00 stamp duty by Inland Revenue Board of Malaysia / 2nd Class Magistrate / Commissioner of Oath) |
||
| 5) | BK Form (Sponsor's Particular Form) |

| 6) | Certified copy of Passport / Travel Documents (all pages) [Note: Copy of previous Passport is required if applicant has renewed his / her Passport in less than one (1) year] |
||
| 7) | Five (5) copies of passport-sized photographs |
||
| 8) | Certified copy of Sponsor's Identity Card |
||
| 9) | Cover Letter from Sponsor |
||
| 10) | Certified copy of medical report and RB II Form | ||
| 11) | Certified copy of Marriage Certificate (if accompanied by spouse) |
||
| 12) | Certified copy of Birth Certificate / Legal Documents (if accompanied by children) |
||
| 13) | Certified copy of Fixed Deposit Statement from any bank / financial institution in Malaysia [RM150,000.00 (couple) & RM100,000.00 (Individual)] |
